Eric C. Rackow, M.D. heads SeniorBridge and Humana Cares as an integrated organization and is responsible for the growth and continuing success of the division.  Additionally, Dr. Rackow serves as a Professor of Medicine at the New York University School of Medicine. 

Prior to his current role, he was president and CEO of SeniorBridge, where under his leadership the company tripled in size to 44 homecare offices and a nationwide network of more than 2,000 care managers coast to coast.    Dr. Rackow joined SeniorBridge following a career in the hospital industry, where he saw first-hand, the challenges frail seniors face in their homes, and the need to provide personalized, ongoing support.  There he became passionate about the effectiveness of care management in averting unnecessary hospitalizations and emergency room visits and its power to improve efficiencies in healthcare.   

Dr. Rackow’s hospital roles include immediate past president of NYU Hospitals Center, where he was responsible for ensuring the quality of medical services and promoting continued excellence in patient care, medical education, and clinical research. Previously, he served as Chief Medical Officer at NYU Hospitals Center and Chairman of the Department of Medicine at St. Vincent’s Hospital and Medical Center of New York.

As an expert in critical care and health delivery, Dr. Rackow is frequently quoted in national press including The Wall Street Journal and The New York Times and frequently speaks at healthcare conferences on aging, complex chronic care, and healthcare delivery systems.
 
After earning his MD at the State University of New York, Downstate Medical Center, he trained in Internal Medicine, served as Chief Resident in Internal Medicine and completed a fellowship in Cardiology at Downstate. Dr. Rackow is the author of 184 articles and 40 chapters on the care of patients with complex medical problems.  He is currently on the Board of Trustees of the Weil Institute of Critical Care Medicine which is dedicated to education and research in caring for patients with severe illness or injury, as well as serving on the Board of Board of Trustees of his alma mater, Franklin and Marshall College. Dr. Rackow is a fellow of the American College of Physicians, American College of Critical Care Medicine, American College of Cardiology, and American College of Chest Physicians. Recently, he received the distinguished award of Mastership of the American College of Physicians.

NAPGCM CertifiedMs. Fine is a pioneer in the field of geriatric care management and  has served in industry and community leadership roles throughout her 30-year career in elder care. Among them, she served as one of the first presidents of the National Association of Professional Geriatric Care Managers.  Prior to joining SeniorBridge, she was a partner with the New York City-based Fine & Newcombe Associates, which was acquired by SeniorBridge in 2001. In addition to her role at SeniorBridge, Ms. Fine is a member of the New York Society for Clinical Social Work Psychotherapists, the American Society on Aging and sits on the Women in Health Management Board of Directors.  Ms. Fine has a Master's Degree in Public Health from Columbia University School of Public Health and a Master's Degree in Social Work from Hunter College School of Social Work.   She is the recipient of numerous industry accolades including the Hunter College School of Social Work Award for Outstanding Service to Social Work in Aging and is frequently featured in national media about topics related to aging and care management.Kathy Driscoll, RN, CMC, Senior Vice President and Chief Operating Officer

She has held a variety of senior management roles including operational, clinical and business development responsibilities. Most recently she was Vice President of Business Development for Premier Home Health Care Services Inc., a multi-state homecare provider based in White Plains, NY. She was previously Vice President of Operations and New Business at SeniorBridge and rejoins the company as Senior Vice President of Operations. Ms Driscoll has a Bachelor of Science in Nursing from Seton Hall University. She is a Registered Nurse licensed in New York, New Jersey, Florida and Connecticut, and is a current member of the National Association for Professional Geriatric Care Managers, The Case Management Society of America, Women in Health Management of NY, and The American Association of Managed Care Nurses.

Prior to her role at SeniorBridge, she worked for 15 years in the Homecare industry at Patient Care, Inc. where she was the Vice President of Quality Management. Her responsibilities included the development of client service standards to ensure that quality care was being delivered and facilitating survey evaluations by accrediting agencies such as JCAHO, CHAP and the Department of Health. She is licensed as an RN in New York, New Jersey, Illinois and Florida. Ms. Morrison graduated from St. Vincent's Hospital School of Nursing in NYC and she received a BSN and MA from Kean University. She is a care manager certified (CMC)certified case manager (CCM) and a member of the National Association of Professional Geriatric Care Managers and the Case Management Society of America.

NAPGCM CertifiedRona Bartelstone is a Licensed Clinical Social Worker, a Care Manager Certified, and a Diplomate in Clinical Social Work. A Cum Laude graduate of the University of New York at Buffalo, Rona earned her Masters in Social Work at Indiana University. She has taught the psychology of aging, healthy aging and other courses on coping with aging parents for adult education of the Dade County School Board, Miami Dade Community College, and Broward Community College.

Active in many community and professional organizations, Rona is a founder and past President of the National Association of Professional Geriatric Care Managers (GCM). She currently is a founder and Vice President of the National Academy of Certified Care Managers (NACCM), an independent, non-profit, credentialing organization. Rona was a Board member of the Greater Miami Alzheimer’s Association Chapter and Chaired the Annual Educational Forum. In addition, she is past Vice President and Treasurer of the Florida Chapter of the National Association of Social Workers. Rona is a member of the National Council on the Aging and a delegate to the National Institute on Community Long Term Care (NICLIC), the Gerontological Society of American and the American Society on Aging.

Michael has extensive, diverse experience as a senior finance and administration executive. He previously served as Chief Financial Officer for Brunschwig & Fils, Inc., Advance Group, Inc., and the Pinnacle Alliance Division of Computer Sciences, Inc. He has also served as Vice President of Finance for Calvin Klein, Inc., Corporate Controller for Coach Inc., and Assistant Controller for the Food Service Division of Kraft General Foods. Michael has a track record in mergers and acquisitions, integration of companies, process improvement, upgrading information technology reporting, strategic financial planning, business development, as well as working with service organizations. He received his BA in Economics and BS in Finance from Fairfield University, and an MBA in Finance from Fordham University.

Angela Handa is a seasoned sales executive with nearly two decades of experience growing healthcare businesses. Prior to joining SeniorBridge, she served as Vice President of Sales and Service for Health First Health Plans (HFHP), a licensed HMO that provides Commercial, Medicare Advantage, and ASO plans to more than 63,000 members. As a key executive of the company for more than 14 years, Angela was responsible for sales, marketing/ advertising, underwriting, customer service, new product development, and participated in directing operations of the corporation. Prior to her career as a managed care executive, she founded D. & H. Insurance Associates, Inc. a consultancy specializing in assisting employers with their retention and compensation goals through strategically designed employee benefit packages. A cum laude graduate of Boston University with a dual Bachelor of Arts in Economics and Psychology, Angela earned her Certified Financial Planner (CFP) accreditation through the College for Financial Planning in Denver. She holds Florida licenses for life and health and general lines and completed America's Health Insurance Plans Certified Managed Care Executive (CMCE) program as well as a Certified Employee Benefits Specialist (CEBS) certification.

With more than 25 years of clinical and executive experience in the health care industry, Catherine combines her background in nursing and public health with her long history of accomplishments working with managed care organizations to provide leadership and direction for SeniorBridge's Care Management programs. Her 13 year tenure as consultant, manager and director within the health care practice of PricewaterhouseCoopers provided her with myriad opportunities to successfully lead complex medical management consulting engagements for multiple health care organizations including the Department of Veterans Affairs, Empire Blue Cross Blue Shield of New York, Memorial Sloan-Kettering Cancer Center, Humana, Kaiser of the Northeast and GHI. The extensive reach of her career includes work as Deputy Executive Director for NYCLIX, Inc., a Regional Health Information Organization in New York City, where her position encompassed duties as Privacy Officer, overseeing operations and w ebsite content development. Among Catherine's career highlights, she has served as Regional Director of Clinical Quality Improvement for HealthNet of the Northeast and as Director of Clinical Quality Improvement and Clinical Innovations for Horizon Blue Cross Blue Shield of New Jersey. Catherine earned her Master's in Public Health with honors focusing on health policy and administration from Columbia University. She also holds a Bachelor of Science in Health Service Administration, magna cum laude, from the City University of New York and a New York State license as a Registered Nurse.

She was the Administrator of an inpatient and outpatient comprehensive Rehabilitation and Orthopedic Division at Downstate Medical Centers in Brooklyn. She has an extensive background in home care marketing and management having worked for St. Vincent’s Catholic Medical Centers as the Director of Intake and was then the Branch Manager/Administrator at New York Health Care. Ruth was also the Executive Director of Sunrise Assisted Living of East Meadow where she was responsible for the daily operations of the facility, client and family satisfaction, management of the sales and marketing team and coordinated community outreach and events. Ruth has a Bachelors of Science from SUNY College of Oneonta where she majored in Business Economics and she is currently completing a Masters Degree at Long Island University specializing in Gerontology. Ruth has received her certification in Geriatric Care Management (CMC). She is a current member of the National Association of Professional Geriatric Care Managers and the Case Management Society of America.

Timothy M. Hanold, Vice President, Sales and Marketing

Timothy is a seasoned business development professional who has worked in the corporate healthcare, automotive and consumer goods industries. He currently leads the national private pay sales at SeniorBridge.  Before joining SeniorBridge, he drove large scale hospital contracts at Covidien Healthcare (previously Tyco Healthcare), a leading international healthcare company and served as a national field sales development trainer.   At Ford Motor Company, Timothy outpaced sales quotas and corporate objectives and set records for client satisfaction. His experience includes developing market share opportunities with Pepsi Co.  He has a Masters degree in Business Management from Columbia University and has a Bachelor of Science degree in Marketing and Business Logistics from Pennsylvania State University.Howard Levine, Vice President, Chief Information Officer

Prior to joining SeniorBridge, Howard was the Director of Information Technology for Loews Cineplex Entertainment in New York where he was responsible for application development and operation support activities. Prior to Loews, he was a Consulting Manager with Ernst & Young, LLP, and Senior Manager with International Systems Services. Mr. Levine holds a Bachelor of Science Degree in Information System Services from Stockton State College in New Jersey.

 

Shira Loyer, Vice President, Communications and Marketing

Ms. Loyer has deep expertise earning leading national and international companies recognition in national media outlets, conferences and key channels. Through her success developing and implementing communications and marketing strategies, she has achieved substantial visibility for SeniorBridge among influencers, partners and clients. Prior to SeniorBridge, Ms. Loyer developed and implemented communications and marketing strategies for brands including Microsoft, Coca Cola, Merck and Novo Nordisk at companies including Waggener Edstrom and Chandler Chicco Companies. Her track record of encouraging happy customers and clients to share their positive experiences in online reviews dates back to 2001 when social media was blossoming. Prior, she worked as a manager of corporate communications in the Silicon Valley to successfully launch technology software companies including AppStream, now a subsidiary of Symantec Corporation. Ms. Loyer is an award-winning writer whose work has been featured in national and local publications. She received her Bachelor of Arts in Journalism from San Diego State University in California.

David Mattioli, RN, CMC, BSN, Vice President, Regional Operations

NAPGCM CertifiedDavid Mattioli brings more than 17 years' experience in a variety of healthcare settings to SeniorBridge. For 9 years previously, he was the Administrator of Almost Family in West Palm Beach, responsible for operations and delivery of care for the in-home program in 4 counties, and for a time served dually in that role at Almost Family's Medical Adult Day Center in Boynton Beach. Mr. Mattioli began his nursing career at St. Joseph Mercy Hospital in Ann Arbor, Michigan, where he developed a passion for caring for the elderly, and became a care manager before returning to Florida. He is a certified trainer for the Alzheimer training course for home health agencies in Florida and coauthored the curriculum used for the training. Mr. Mattioli is a member of the National Association of Professional Geriatric Care Managers. He received a BS in biology from Palm Beach Atlantic University and a BSN from Eastern Michigan University, and is a certified Legal Nurse Consultant. He is a current member of the National Association of Professional Geriatric Care Managers and the Case Management Society of America.

Jim Resciniti, CPA, Vice President of Finance

Jim Resciniti brings a wealth of experience, managing finance operations in profit and nonprofit organizations and in public accounting. Jim was Controller and then Assistant Vice President of Finance for one of New York’s largest hospital based managed care companies where he spent ten years as a key finance leader cultivating a superior business environment through continuous process improvement, for a diverse healthcare organization growing to over $1 billion in revenue with an administrative budget of more than $150 million. Prior to that, he was Accounting Manager for Manhattan’s historic Parish of Trinity Church, and Audit Manager in public accounting where he worked closely with hospitals and nursing homes for more than six years. He earned a BS in Accounting from Brooklyn College and New York State CPA licensure.

Michele Rosenblum
Vice President of Homecare Services

Michele brings more than a decade of healthcare experience with particular expertise in operations management and new business development. Her strong skill-set has allowed Michele to develop new home health care business lines, expand market share and provide strategic marketing for multi-branch, multi state companies. Prior to joining SeniorBridge, Michele served as a Health Care Consultant where she focused on managed care including budget preparation and projections, network development and application processing for participation in New York Medicaid managed care. Other career highlights include directing daily operations of Medicare certified home health agencies as Area Vice President of Gentiva for Long Island, Metropolitan and Capital District of NY, and developing home care agencies for Erickson Retirement Communities. Michele has also held a prior position at SeniorBridge where she served as Executive Vice President of Operations during the start-up of the company’s business operations. Michele earned her Bachelor of Arts degree in Sociology from Misericordia University in Dallas, Pennsylvania and her Bachelor of Science degree in Nursing from Adelphi University in Garden City, New York. Michele is a former member of the Board of Directors of the Home Care Association of New York and a founding member of the Home Health Care Association of Washington, D.C. Michele holds licensure in the state of New York as a Registered Nurse.

Mr. Sosnow joined SeniorBridge in 2000. Before joining SeniorBridge, Mr. Sosnow held the position of Administrative Sales Director with Advica Health Resources, a national management services company administering health services to over 200,000 people. While with Oxford Health Plans, Mr. Sosnow was responsible for regional contracting with hospital, physician and ancillary health providers. Mr. Sosnow is a graduate of Denison University.

NAPGCM CertifiedBefore joining SeniorBridge, Kristi spent 13 years with Patient Care, Inc. in Essex County, most recently as Vice President of Risk Management. Her responsibilities in this position included infection control and safety program development, as well as insurance oversight and claims management. Previous clinical experience included over 10 years in obstetric, emergency room, surgical and infectious disease nursing. Ms. Catallo holds licenses as a Registered Nurse in New York and New Jersey and is a member of the American Society of Healthcare Risk Management. She is an active member of the National Association of Professional Geriatric Care Managers. Ms. Catallo is a graduate of the State University of New York at Alfred.

Judy Sweigart, RN, Quality Management Director

With more than 30 years' experience in hospital and home health care, Judy is eminently qualified to understand the needs of our clients and staff. She comes to SeniorBridge from Patient Care, Inc., in West Orange, NJ, where she was involved in documentation review and utilization with the nursing staff and therapists. Previously at Patient Care, Judy was a Home Care Liaison between Case Managers and Patient Care as patients transitioned to homecare from the hospital. She was also a Quality Management Consultant there, auditing clinical records of a large Medicare- certified home health agency, providing corrective action programs, and instructing nurses in documentation and quality management issues. Judy has also instructed home health aides in patient care. She has been a Case Manager for Visiting Nurse Services of Elizabeth and Staff and Charge Nurse in various units at Union and Columbus Hospitals in New Jersey. Judy is a licensed RN who obtained her credentials at Orange Memorial School of Nursing, affiliated with Montclair State University, and completed a Certificate in Legal Nurse Consulting at Fairleigh Dickinson University.
