Appropriate care delivery requires a more comprehensive and coordinated view of the individual. SeniorBridge provides home-based care management programs that are especially beneficial to those with significant chronic, and often times multiple, medical conditions. Alzheimer's disease, Parkinson's disease, congestive heart failure (CHF) and chronic obstructive pulmonary disease (COPD) are of particular note in this regards. The company's geriatric care mangers lead coordinated care delivery teams to provide a range of in-home services customized for the unique needs of each individual.
Unlike most home care service providers, SeniorBridge has to date operated largely under a retail / private pay model suggesting a perception among seniors of meaningful value for the Company's programs and high-touch service market (via health plan sponsors) as the need for more effective chronic care management in the home is recognized.
