Kristi Catallo, RN, CMC, CCM
Vice President of Regional Operations

Kristi Catallo joined SeniorBridge in May 2004. Before SeniorBridge, she spent 13 years with Patient Care, Inc. in Essex County, most recently as Vice President of Risk Management. Her responsibilities in this position included infection control and safety program development, as well as insurance oversight and claims management. Previous clinical experience included over 10 years in obstetric, emergency room, surgical and infectious disease nursing. Ms. Catallo holds licenses as a Registered Nurse in New York and New Jersey and is a member of the American Society of Healthcare Risk Management. She is an active member of the National Association of Professional Geriatric Care Managers. Ms. Catallo is a graduate of the State University of New York at Alfred. 

Nancy Cullen
Director of Professional Relations

Nancy joins SeniorBridge with more than 30 years experience in the healthcare delivery field. Most recently, Nancy was the Director of Provider Relations at Acuity Healthcare, where she successfully built professional relationships within the community while advocating for high quality patient care. Previously, she worked for more than 25 years with the Carolinas Healthcare System, and was instrumental in guiding the daily operations within the acute rehabilitation and home health care settings including community relation building, liaising with physicians and discharge planning. Her many years within the healthcare system provide her with a broad depth of experience. Among her accolades, Nancy earned the President's Award for Excellence in Action and a Certificate of Appreciation from ADAM (a disabled men's group). She holds a Master of Science in Recreation Resource Administration from North Carolina State University.

Tammy Harrison, RN
Care Manager

Tammy brings 7 years experience in the healthcare field with particular expertise in home care and education to her role as Care Manager and her commitment to excellence encompasses all areas of her work including strong client care and advocacy. Prior to joining SeniorBridge, Tammy served as Registered Nurse for Carolinas Center for Medical Excellence where she visited Medicaid recipients and potential recipients in home to collect data regarding eligibility for services, provided information to recipients about home health agencies in the area and interpreted and applied Medicaid clinical policies. Other career highlights include performing initial and quarterly in-home patient assessments as RN for Care Pro Health Services and caring for the ill, injured or convalescing persons with disabilities as a Licensed Practical Nurse for Park Pointe Village, a skilled nursing home. Tammy earned her Associates degrees for Medical Laboratory Technician, Licensed Practical Nurse and Registered Nurse all from York Technical College. Tammy holds licensure in the states of North and South Carolina as a Registered Nurse and holds certifications as a BLS Healthcare Provider and a Medical Laboratory Technician.

Frances Lane, RN
Care Manager

Frances brings more than 35 years of professional nursing experience to her role as Care Manager with particular expertise in adult medicine and the home care field. Her commitment to excellence encompasses all areas of her work including client care and advocacy. Prior to joining SeniorBridge, Frances served as staff nurse for Homewatch Caregivers, a well-regarded company in the Charlotte area, where she was responsible for assessments and supervisory visits, provided instruction of personal care aides and assisted clients and families in the management of their care. Other career highlights include working on medical and oncology units as staff nurse for Northeast Community Hospital in Texas and providing intravenous therapy and wound care as a visiting nurse for Morris County Visiting Nurse Association in New Jersey. Frances has an extensive background in nursing and also has experience in physical therapy, the emergency room and patients with cardiovascular and diabetic issues. Frances graduated from the College of Allied Health Professionals at Temple University. Frances holds licensure in the states of Illinois and North Carolina as a Registered Nurse.

Mary Medford, RN, BSN, MA
Care Manager

Mary brings 30 years of professional nursing experience in the healthcare field with particular expertise as an educator and in homecare. Her commitment to excellence encompasses all areas of her work including strong client advocacy and has experience in a variety of settings including critical care, medical surgery, ER, home care, parish nursing and hospice. In Mary's nursing career she has worked for several long-term care insurance companies as a long term care underwriter specialist and multiple home care agencies and hospitals throughout California and North Carolina. Prior to joining SeniorBridge, Mary served as a home health nurse and Case Manager in the Charlotte Region. Other career highlights include RN Case Manager and Admissions Nurse at Community Home Care & Hospice, and Staff Registered Nurse at Presbyterian Matthews Hospital and Anson Community Hospital, Wadesboro. Mary has also worked as a Certified Nurse Assistant (CNA) I and II and Nutrition Instructor for several years at the community college level, holds a Bachelor of Arts in Psychology, Bachelor of Science in Nursing and a Master of Arts in Intercultural Studies. Mary holds licensure as a Registered Nurse in the state of North Carolina and certifications in Advanced Cardiac Life Support (ACLS) and Pediatric Advanced Life Support (PALS) and as a Basic Life Support (BLS) instructor.

Jennifer Parker, MSW, LCSW
Care Manager

Jennifer is a licensed clinical social worker with more than 20 years of experience providing social work services to individuals and families in healthcare, nonprofit and educational settings. For 11 years, Jennifer worked as a clinical case manager at Carolinas Rehabilitation, serving on the stroke and  traumatic brain injury teams and helping families adjust to new roles in caregiving and the disabilities of loved ones. As a Care Manager, her professional approach includes a commitment to enhancing the quality of life by empowering  clients and their families through compassionate care, supportive education and quality community and professional resources. Prior to joining Senior Bridge, Jennifer has worked as a counselor and as an award-winning presenter and professional development facilitator. Jennifer received her Bachelor of Arts and her Master of Social Work degrees from the University of North Carolina at Chapel Hill, and a Master of Arts degree from The University of Chicago.  She holds leadership roles in alumni initiatives of both universities, and has served on the boards of several nonprofit agencies in the Charlotte Metro region. Jennifer is a member of the Board of Advisors for the School of Social Work at the University of North Carolina at Chapel Hill. As a member of the National Association of Social Workers (NASW), Jennifer was recognized by the North Carolina Chapter of NASW for contributions to professional development in the field of social work.

Wendy Tener, RN, BSN, CCM
Care Manager

Wendy brings more than 14 years of professional nursing experience in the hospital, community and corporate settings and has particular expertise in critical care and chronic disease case management. Prior to joining SeniorBridge, Wendy served as Senior Disease Case Manager for CVS/Caremark-Accordant Health Services where she managed a caseload of approximately 40 rare chronic disease health plan members and coordinated care, assisted with formulation of post-discharge and transition of care assessment tools and developed educational materials related to medication compliance. Other career highlights include completing staff in-service and updates on Multiple Sclerosis research and development in a Disease Manager/Analyst position at Accordant Health Services and previous critical care nursing experience at High Point Regional Health System.  Wendy attended Greensboro College in North Carolina where she was biology major. She then went on and earned a Bachelor of Science in Nursing degree from University of North Carolina at Greensboro and has completed some master’s degree work in the Masters of Science in Nursing/Healthcare Systems Management program at Loyola University New Orleans. Wendy holds licensure in all fifty states as a Registered Nurse and also holds certification as a Certified Case Manager.

Sabra Watkins, RN, MPH
Care Manager

Sabre brings more than 15 years of experience in the healthcare field with a deep commitment for ensuring quality of care given to clients, as well as advocating for families. Prior to joining SeniorBridge, Sabre served as Supervisory Nurse at Partners In Care, a comprehensive home care agency in Charlotte, North Carolina. Throughout her time there, Sabre performed home health care assessments and supervisory visits for clients, interviewed clients about their current needs and supervised and coached certified nursing assistants. Other career highlights include planning all employee safety training and maintain employee records, incident reports and training logs as Employee Safety Coordinator at United Hospital in St. Paul, Minnesota and enrolling participants in studies and following up with clinics and hospitals of participant’s diagnosis as Student Worker for the Minnesota Department of Health. Sabre also has an extensive background in education and environmental studies and has taught students the general concepts of environmental health and physical science and developed a curriculum for learning delayed students in Greensboro, North Carolina and educated students on the basics of biology while in Minneapolis, Minnesota. Sabre earned her Bachelor of Arts degree in Biology from University of North Carolina at Charlotte, her Bachelor of Science in Nursing degree from Queen’s University of Charlotte and her Masters in Public Health- Environmental Health from University of Minnesota School of Public Health. Sabre holds licensure in the state of North Carolina as a Registered Nurse.
