Sherry S. Kostman, Psy.D, MA, CADC, MISA, NCRS, CCM
Regional Executive Director
Before joining us, Dr. Sherry Kostman was Director of Psychosocial Rehabilitation Services at a psychiatric Long-Term Care Facility and Divisional Director at Abraxas Youth & Family Services-Cornell Interventions, where she had full clinical and fiscal responsibility for 25 residential and outpatient behavioral health and substance abuse programs across Illinois. Dr. Kostman earned a Doctorate of Clinical Psychology and Certificate in Group Psychotherapy from the Adler School of Professional Psychology in Chicago, and an MA in Clinical Psychology from the Illinois School of Professional Psychology. She is a Licensed Clinical Psychologist in Illinois and South Carolina, as well as a Certified Alcohol and Other Drug Abuse Professional, Certified Mental Illness/Substance Abuse Professional, a National Certified Recovery Specialist and a Certified Case Manager. Dr. Kostman is also active in the local community and serves on the board of the Midwest Chapter of the National Association of Professional Geriatric Care Managers. Prior to entering the healthcare field Dr. Kostman was Executive Vice President of Kostman/Schmid Advertising for over 18 years.
Christine Martinez
Executive Director
Christine has more than 17 years experience in the healthcare field with an extensive background in healthcare administration, business development, management, and mentoring and team building. Her strong skill-set has allowed Christine to develop long-term relationships and strategically set up business plans throughout the South Texas area, where she is known for her high levels of integrity and leadership. Prior to joining SeniorBridge, Christine served as Executive Director at BrightStar in San Antonio where she oversaw the branch and territory, planned business strategies, implemented policies and procedures and was responsible for weekly sales, marketing and education in the community. Other career highlights include collaborating with branch managers to develop in-house sales and recruitment as Director of Sales and Marketing at ResCare Home Care and developing business in each market by supporting local sales force as Area Director and Administrator for Nursefinders Home Care. Christine attended Northwest Vista College where she took general studies with business emphasis.
Tami Copeland, RN
Director of Client Services
As the Director of Client Services for the San Antonio care management team, Tami provides clinical leadership and mentoring with more than eight years nursing experience working with complex medical cases. During her five-year tenure as a Care Manager with SeniorBridge, Tami expertly developed client-centered care plans by facilitating services designed to improve quality of life for clients and relieve stress for families. She continues this approach to care management and takes pride in meeting the individual needs found within each case as she inspires the San Antonio team of caregivers and care managers with her passion for the field of geriatrics. Tami's previous work includes serving as a Charge Nurse and Surgical Floor Nurse at McKenna Memorial Hospital in New Braunfels where she, among her other clinical duties, provided chemotherapy treatments as a certified provider. Her broad scope of experience also includes founding her own customer service business and 15 years in business management, recruiting, training and consulting. Tami earned her associate's degree in nursing from San Antonio College and is currently continuing her education there in pursuit of her bachelor's and master's degrees in nursing. She is licensed as a Registered Nurse in the State of Texas.
Nick Escobar
Director of Professional Relations
Nick brings healthcare marketing and sales experience, with particular expertise in new business development and territory management, to his role as Director of Professional Relations. Prior to joining SeniorBridge, Nick served as Director of Marketing and Business Development at Home Instead Senior Care where he developed market and territory awareness in San Antonio, maintained relationships with referral sources and increased sales revenue. Other career highlights include serving as Senior Recruiter for Maxim Healthcare Services and sales associate for Sewell Automotive Co: Ken Batchelor Cadillac. Nick earned his Bachelor of Business Administration specializing in management, marketing and general business from Texas Tech University in Lubbock, Texas. While there, he was an active member of the school community and founded The Forum for Leadership Excellence and Tech Elite Association of Management (TEAM). Nick was also a member of Sigma Iota Epsilon- the Management Honor Society, Tech Marketing Association (TMA), Hispanic Student Society and Texas Tech University Intramurals.
John Anderson, LMSW
Care Manager
John has worked for social service organizations for more than 24 years. Since 2003, he has been a psychotherapist with Senior Connections of Texas. Previously, he was a hospital-based Adult Protective Services Specialist for the Texas Department of Protective and Regulatory Services, coordinating APS client services with with the University and VA hospitals. Earlier, he investigated reports of abuse and neglect of elderly and disabled clients for the state and consulted on improving quality and intensity of client services. John has experience as a Social Worker serving community-dwelling elderly and disabled adults, CPS worker, child care licensing representative, Medical Specialist and Instructor in Psychiatric Procedures for the US Army at Brook Army Medical Center at Fort Sam Houston. He completed a Bachelor of Arts degree in Psychology at the University of Florida, Gainesville, and has been a Licensed Master Social Worker for over 20 years.
Lebeka Flom
Home Care Supervisory Nurse
Lebeka's commitment to excellence encompasses all areas of her work as Care Manager including strong patient care and advocacy with experience in the hospital and rehabilitation settings. Prior to joining SeniorBridge, Lebeka served as License Vocational Nurse (LVN) for Warm Springs Rehab Hospital and her other career highlights include serving as LVN for Global Rehab of San Antonio and LVN for The Haven & Laurels of Stone Oak. Lebeka graduated from St. Phillips College and gained practical experience from Baptist School of Health professionals and Galen College of Nursing.
Terry Ann Harris, RN,BS, MA
Care Manager
Terry Ann brings more than 30 years of professional nursing experience in a variety of settings including hospitals, medical centers and hospice care. Her commitment to excellence encompasses all areas of her work as Care Manager including particular expertise in acute patient care and family advocacy. Prior to joining SeniorBridge, Terry Ann served as Case Manager at Amedisys and Lighthouse Hospice where she cared for patients with terminal illnesses and promoted a palliative approach of care including physical, spiritual and psychosocial support for patients and families coping with illness. Other career highlights include facilitating the focus of a historically social work-driven case management system to a Registered Nurse-driven team as Director of Care Coordination at Peterson Regional Medical Center and developing a Community Partners Program for the uninsured and indigent patients in rural New Mexico as Director of Case Management at Alta Vista Regional Hospital. Terry Ann earned an Associate Degree in Nursing from Mount Calm Community College, a Bachelor in Science degree in Community Development with a minor in Health Science Administration from Central Michigan University and a Master of Arts in Rehabilitation Counseling from Michigan State University. Terry Ann holds licensure in the state of Texas as a Registered Nurse and holds certifications in Basic Cardiac Life Support (BCLS), NMCC and CRC.
Melinda (Mindi) Silver-Weiss, MSW, LCSW
Senior Care Manager
For more than 27 years, Mindi has been a social worker involved in varied facets of healthcare. She was formerly a crime Victim Advocate for the Jewish Family Service in San Antonio and also Associate Director of the Dorothy & Peter Brown Jewish Community Adult Day Care Program in West Bloomfield, MI, supervising staff and providing services to participants with dementia and their families. Previously, Mindi also served as Supervisor, Senior Services, and Clinical Social Worker at the Jewish Family Service in Southfield, MI. She has been a Nephrology Social Work Coordinator at the Southfield Dialysis Facility and Nephrology Social Worker at William Beaumont Hospital in Royal Oak, MI, and Hutzel Hospital in Detroit. Mindi received a BA in Sociology/Social Welfare from Wayne State University and her Master of Social Work degree from the University of Michigan School of Social Work, Ann Arbor. She is a member of the Academy of Certified Social Workers, and is licensed in Michigan and Texas.
Bridgette Woods, RN
Care Manager
Bridgette brings more than 17 years of nursing experience in the healthcare field in a variety of settings including hospital, home care and clinical office. Her commitment to excellence encompasses all areas of her work as Care Manager including particular expertise in acute patient care and family advocacy throughout the San Antonio area. Prior to joining SeniorBridge, Bridgette served as Staff Nurse for BAMC, an internal medicine clinic, where she provided patient care, supervised 9 Registered Nurses and 7 Licensed Vocational Nurses and assisted with interviewing nursing personnel. Other career highlights include performing basic and urgent care as Contract Staff Nurse at Spectrum Healthcare and providing primary care to cardiac rehab patients as Staff Nurse and Weekend RN Charge Nurse as Metropolitan Methodist Hospital. Bridgette earned her Licensure of Vocational Nursing from St. Phillips College and her Associates in Science/Nursing degree from San Antonio College in Texas. She holds licensure in the state of Texas as a Registered Nurse.
