Former Home Health Patients of Humana At Home (Dallas), Inc. and Humana At Home (Houston), Inc., to obtain your medical records:

Humana At Home (Dallas), Inc. and Humana At Home (Houston), Inc. (collectively “HAH Dallas/Houston”) have ceased operations.  If you are a former patient and wish to obtain a copy of your medical records of your care with either agency, we will be happy to transfer a copy of your records to either you or to any provider of your choice. We require that you complete the form at this link to give us permission to send the record, as well as the name and address of the person to whom you wish the record sent.  Once you complete the form, you can either send it via email to the following email address: MedicalRecordCustodian@Humana.com or by regular mail to Medical Records Request, 845 Third Avenue, 7th floor, New York, N.Y. 10022.  Please ensure that you give us a way to reach you in the event that the form is not complete, as we can only release records if the form is signed and complete.  There is no fee for the first copy of your records.  If you request more than one copy, there may be a cost-based fee, but you will be notified of any fee before your request is processed. You or your provider should receive your record within 30 days of receipt of your request.
