Concetta Pereira, a SeniorBridge Social Work Field Care Manager, has been recognized with a 2020 Summit Award for her outstanding work.

The Summit Award formally recognizes engaged, exemplary individuals who consistently go above and beyond in creating the Humana “Perfect Experience” for those we serve (including each other) to help people achieve their best health and lifelong well-being.
"The services that SeniorBridge provides, it's seen as the extended family, extended village, says Concetta. “It takes a village to raise a child, but it also takes a village to support the elderly. "The Perfect Experience means that the person has the best quality of life that they can have at this point in their lives...they have some joy,” she adds. “And I will look for joy. I will look for programs that will enhance somebody's life.”
Ruth, one of Concetta’s SeniorBridge clients, agrees. "You can't really talk about a Perfect Experience with Concetta,” she says, “because Concetta is an ongoing experience in your life. She's there for you…I have never quite met anybody as kind, as generous of her time, her talent. I cannot say enough about what a tremendous human being this is. I feel very privileged that she is in my life."
Ruth is an independent, "self-made woman" as described by herself and others. Throughout her career, she was a social worker, an actress, a film producer, and most importantly, a mom to her only son, Larry. More than 30 years ago, she purchased a brownstone in Brooklyn and moved there to raise him.
After an extended hospital stay, Ruth’s family worried about her health, and whether it was safe for her, now 91, to stay at home on her own. They reached out to SeniorBridge—and Concetta. Says Larry, "We heard, ‘oh, there’s only one place you should call—SeniorBridge.’ I certainly can't imagine doing all of the things that Concetta and SeniorBridge do without them."
As a SeniorBridge Social Work Field Care Manager in Behavioral Health, Concetta is responsible for providing care management to geriatric clients in their homes in collaboration with the nursing staff. She conducts psychosocial assessments and is responsible for developing and implementing the social work perspective within individualized care plans.
In any given day, Concetta might coordinate medical treatment, attend doctor appointments, assist with long-term planning, provide supportive counseling, connect clients to resources/entitlements, and provide opportunities for social engagement.
She also works through client resistance to care while monitoring their safety and well-being in the community, advocating on behalf of clients and their families. Concetta carries a caseload of complex clients in whose lives she is ever-present. She has made it her life's work to support, care for, and act as extended family for them.
